- Passport bio-data page; passport valid at least 7 months from expected arrival in Malaysia.
- Recent passport photograph with a white background.
- Return flight ticket within 30 days of arrival.
- Hotel booking, or Malaysian host sponsor letter with Malaysian ID and a recent utility bill.
- Last 3 months bank statements showing sufficient funds for the stay.
- Typed and signed visa request letter.
- Employment letter; self-employed applicants: business registration or proof of income.
- Applicants under 12: birth certificate copy.
